You will need to complete and submit the enrollment packet along with all payable fees. Select whether your child will be enrolled as a full-time or part-time student. If you are changing from part-time to full-time , a Change Of Registration form is required. The change becomes valid only when you complete this form and submit it to the office. All forms must be complete and signed on or before the first day of care. The following forms and information are required for enrollment:

• Physical Examination Consent for Medical Treatment Form
• Immunization Record Emergency Release Form
• Copy of Child's Birth Certificate Emergency Information Sheet
• Social Security Card Acknowledgement of Receipt
• Health History

RSA shall not discriminate against any child or parent because of race sex, or political affiliation. Parents will have the opportunity to observe programs during the time of operation before registration. At the time of enrollment, parents will complete the applications forms, read all pertinent information, and turn in shot records, social secrity card, and birth certificate. All indivivdual files shall be kept confidential. A form will be submitted and signed by the parent(s) to release any information from the child's file.

Enrollment terms will be for one year. RSA will use the initial enrollment as the expiration date. Then, a re-enrollment fee is required. A letter or communication form will be sent to the parent one month before the enrollment expiration.

Attendance/ Arrival/ Departure:
All children must attend school daily, except in cases of illness, emergencies, or parent vacations. A doctor's release will be required on special cases of illness in order to return to school ( Measles, Chicken Pox, Hepatitis).
