Ages 3 years to 6 years old $125.00

we accept cash, money orders, and Child Care Resource & Referral (CCR & R). Whatever the Child Care Resource & Referral does not pay, the parent(s) are responsible for the remainder of the rate.

Emergency Information Sheet

Acknowledgement of Receipt

DCFS Summary of Licensing Standards

• Registration Fee (Non-refundable)
• Birth Certificate
• Social Security Card or proof of ordered form
• Current physical ( must be on DCFS form )
• Immunization/Documented Shot Record

 

(Must be on DCFS form along with a copy of shot records)

• TB Skin Test
• Lead Test
